Premium
The amount paid periodically for coverage.
Deductible
The amount you may pay before coverage begins paying for certain services.
Copayment
A fixed amount you may pay for a covered service or prescription.
Coinsurance
A percentage of a covered cost that you may be responsible for.
Network
The doctors, hospitals, pharmacies or other providers participating with a plan.
Formulary
